Schedule of rates template needed

Wonder if anyone help me out here please?

What I'm after is a template for a schedule of rates, which I can use as a starting point for our own schedule of rates here in New Zealand, which have never been developed before.

I'm a former Interserve and RMDer in the UK, now working for Acrow Ltd in Auckland, New Zealand.

Any help would be greatly appreciated.
