If a customer wants a ginny wheel and rope with the scaffold, is it usual to charge extra for this and if so how much? They're only about £50 to buy I think so I might buy one to keep anyway.

I stopped supplying them and chutes etc years ago as they go missing or come back damaged. The local hire shop supply them so they can get it from there.
If you want to supply them try your local HSS hire shop and charge what they charge
