Scaffolder did not wear flotation: inquest

You have got to be kidding me. Is this where we are with safety now?
 
IMK he was using Layher in a cantilever, he hit the brace when swinging for the transom and went down with the gear. A mate I worked with in Oz was on the job with him. A few months later I done a hanger from a jetty, and they tried to force us to use harnesses, I refused to work until I got a PFD and not a harness .
 
I may be missing something, but could you not use both?
 
Think the point may be that he was hooked on to the scaffolding which went down with him, dragging him under. Had he not been appended to the scaffolding and wearing a life jacket, he may have survived.
