QFS Scaffolding Ltd having previously agreed to make public the findings from the investigations and have asked for the publication of the following statement:

The QFS Board of Directors immediately convened an internal investigation into the accident, furthermore the Board agreed to make public the findings of this investigation for the benefit of the industry irrespective of the outcome.

Two further independent investigations have been conducted into the accident. One by the Main Contractor and the second by the H.S.E.

Findings:-

QFS Scaffolding Ltd:
QFS Scaffolding Ltd own exhaustive internal investigation concluded that all current criteria and requirements had not only been met but were exceeded.

It is the decision of the Board of Directors, that whilst we feel we could not have prevented this accident we would await the outcome of the other independent investigations and adopt where possible any suggestions made.

The Main Contractor:
The Main Contactors investigation, which was very complete and in-depth, arrived at the same conclusion as QFS Scaffolding Ltd

H.S.E:
On Monday 1st June 2015, after 5 months of intensive investigations, the HSE has stated that they will not be taking any action against QFS Scaffolding Ltd nor have they made any recommendations.
The H.S.E thanked us for our co-operation during the investigations. Clearly this is a positive note for those directly involved within QFS Scaffolding Ltd. However this vindication in no way diminishes the fact that during the normal daily activities of QFS Scaffolding Ltd operations 2 people were seriously injured.

We have remained mindful of the wellbeing of the two injured parties and the QFS team involved. These people remain at the forefront of our thoughts.

We as a company continue to offer, where possible, support and sympathies to all parties directly involved and wish ALL concerned a full a speedy recovery from the consequences of this accident.

In closing, QFS Scaffolding would take this opportunity to offer our thanks to both the Main Contactor and the HSE for taking a very candid, balanced and open-minded view during the course of all investigations.

This is in no way a statement of complacency, but should serve to demonstrate with clarity that the scaffolding industry remains a potentially dangerous industry, an industry which those at QFS Scaffolding Ltd continue to strive to improve upon and to mitigate its inherent dangers.

Great news qfs but what happened to the scaffolder? And What actually happened ie rope an wheel come loose, brace or loose tube etc to cause the accident?

Lets hope the scaffolder himself can now move with his life. Very nice of QFS for the open honesty.
