Can anyone tell me what a Crusader form is and how to apply for one?

Garry informed me it's an additional sick pay to SSP, I was enquiring for a friend.

its an insurance pot that is on a lot of blue book sites as well as offshore though not all companies are in it.

Never missed a day's graft ever but certainly not on a blue book, too far from home and too much money too drop.

basically a form requesting where to send your payout to your next of kin in the event of a fatality or serious accident,you state who you would like to recieve the payout mate.

Swiftly Garry is correct it is additional sick pay provided by some companies in the scheme! Ask the company they work for about it
