Airlifted man ‘had fallen from ladder’

Really hope the fella makes a full and speedy recovery , but how on earth did he fall of that scaffold it looks spot on to me,
 
It's going to be rife with HSE in the northeast on scaffolds I bet, not that I have any thing to worry about as I ALWAYS do it by the book............ Hmm
 
If you look close half way up the ladder the ladder is resting on a tube which is between two rungs off the ladder.

not saying this cause it he could off fell from anywhere off the ladder.
 
I've always thought it's best practice to have an additional ladder bay, or at the very least, have the ladder coming up through the scaffold.
 
There was a death on site about a year ago on a mcalpine job where a fella fell thru the gap between the top handrail and the ledger as he was climbing the ladder

Think he was carrying tools at the time..... I myself am on a mcalpine job at the mo and whilst carry the cutter I lost my bearings and nearly come off (as I had one hand on the ladder)........ Just shows how easily done it is hope the fella gets well soon ;)
